UCF Listings
The UCF Listing tab shows all submitted forms in a Successful (submitted) status.
For each form, the Created At date and timestamp, State, number of Warnings and Errors, and Status are indicated in the applicable column.
Incomplete forms and forms with unresolved errors will remain in a Pending status, accessible via Manage Claim Forms > Pending Claim Forms, until all required fields are completed, errors are resolved, and the form is successfully submitted to the PDMP.

Manage Claim Forms
The Manage Claim Forms tab shows all Pending and Submitted forms. Upon selecting the Manage Claim Forms tab, the user will immediately be shown all Pending Claim Forms.
UCFs that have been successfully submitted can be viewed in Submitted Claim Forms. To access successfully submitted UCFs, click the Switch to Submitted Forms button.
Pending Claim Forms
Incomplete forms and forms with unresolved errors will remain on the Pending Claim Forms tab in a Pending status until all required fields are completed, errors are resolved, and the form is successfully submitted to the PDMP. To edit a UCF in a Pending status, click the Edit button. To delete a UCF in a Pending status, click the Delete button.

Submitted Claim Forms
Submitted forms can be found on the Submitted Claim Forms tab.
For each form, the Created At date and timestamp, State, and Patient Name are indicated in the applicable column.
To view the details of the submitted UCF, click the hyperlinked name in the Patient Name column to display the Claim Form Summary.
Submitted forms may not be modified in PMP Clearinghouse, only voided/deleted. To void a submitted UCF, click the Delete Submission button.

Voiding a record is a permanent action. Records voided in error will need to be resubmitted to the PDMP. |
