Errors in Universal Claim Forms (UCFs)
UCF data submissions will be immediately validated upon submission. If there are any issues with the submission, PMP Clearinghouse will display the number of errors in a banner at the top of the Edit UCF page and erred fields will be indicated in red.

To save the UCF and resolve errors later, click the blue Save button. Clicking Save does not submit the data to the PDMP. All saved (Pending) UCF submissions will be listed on the UCF Listings tab and available for review/editing on the Manage Claims Form tab. |
All Successful (submitted) UCF submissions will be listed on the UCF Listings tab. Pending (saved) UCF submissions will available for review/editing on the Manage Claim Forms tab. Warnings & Errors in UCF Listings
The UCF Listing tab shows all submitted forms in a Successful status. Forms with unresolved errors will remain in a Pending status, accessible via Manage Claim Forms > Pending Claim Forms, until errors are resolved and the form is successfully submitted.
For each submitted form, the number of Warnings and Errors are indicated in the applicable column.
Addressing Errors & Deleting Forms in Manage Claim Forms
The Manage Claim Forms tab shows all Pending and Submitted forms. Upon selecting the Manage Claim Forms tab, the user will immediately be shown all Pending Claim Forms.
Forms with unresolved errors will remain in a Pending status until errors are resolved and the form is successfully submitted. To edit a UCF in a Pending status, click the Edit button for the UCF.
If a UCF was successfully submitted with errors, the UCF may be deleted in Manage Claim Forms. To delete a successfully submitted UCF, click the Switch to Submitted Forms button. Locate the UCF that requires removal, then click Delete Submission.
UCF Errors, Modification, & Voiding in PMP AWARxE
A UCF with errors cannot be successfully submitted to the PDMP until all errors are resolved. Therefore, error correction through PMP AWARxE would not be applicable.
UCFs that have been successfully submitted but require modification or voiding after the fact may be modified in Rx Maintenance. If enabled by the PDMP for user access, the Rx Maintenance dashboard in PMP AWARxE allows users to search for a specific dispensation record and update or void it after it has been accepted into PMP AWARxE.
