Prescriber and Dispensary Information

1. As a submitter, if the DEA submitted in PHA03 belongs to a Prescriber, then the record will not be subject to Pharmacy License (PHA13) validation.
   
   • This is true for both files submitted via SFTP and Universal Claim Form record submissions.
2. Enter the Prescribing DEA into PRE02 and the dispensing DEA into PHA03.
   • Note: If a Prescriber’s DEA is entered in both PRE02 and PHA03, then PHA13 is not required.
   • On the Universal Claim Form, you will enter the DEA number into the DEA number field within the Pharmacy and Prescriber sections, as seen below:

     • Note: The demographic information comes directly from the DEA registry
