Prescription Numbers

• If your organization does not use Rx numbers, you may generate a prescription number to enter into the PDMP utilizing the following formula: YYMMDDABC
  • Where YYMMDD indicates the fill date, and ABC indicates the patient’s initials
  • If the patient has more than one prescription for a given date, you can append YYMMDDABC with 1, 2, and so on. Ex:

      or
    • 220317JRC
    • 220317JRC2

• If the patient is an animal, please use the owner’s first name, owner’s last name, owner’s date of birth, and gender. Also ensure you’ve indicated the species for the prescription
  • If the patient is an animal, the user should provide the animal name in PAT 23
  • On the Universal Claim Form in PMP Clearinghouse, you can do this by checking the Patient Animal box:
