PHA11/Contact Name Validation Issue:
We apologize for any inconvenience. You are receiving this error message either because:
- You are submitting for a pharmacy and did not include the license number in the PHA11 (Contact Name) field
- To resolve this issue please enter the pharmacy license number in the field; if you submit via file you may need to contact your vendor to make sure this information is included in every file
- You are submitting for a pharmacy and the license number you provided is not currently in the list of accepted licenses provided by the state of California
- This list is updated on a regular basis and errors are reprocessed on a nightly basis to allow for acceptance of newly added pharmacy license numbers
- You are submitting for a direct dispenser and the pharmacy (dispensing) DEA does not match the prescriber DEA.
- If these values do not match, the error message is expected.
