NDC Errors

This article outlines what to do when you receive an NDC error message upon submission.

NDC Errors on Compound Drugs

If you are submitting a compound drug and receiving an NDC error, reference your state's Clearinghouse Dispensation Guide for specific instructions on how to submit a compound drug.

1. Verify that the NDC you are using is eleven digits long.
2. Verify the format of your NDC.
   • NDCs are formatted in a 5-4-2 grouping (E.g., 12345-1234-12)
   • If your format doesn't follow a 5-4-2 grouping, add a leading zero to whichever section is missing a digit. (E.g., 1234-123-1 should be 01234-0123-01)
3. Ensure that you are not inputting dashes or special characters in the NDC field. PMP Clearinghouse only accepts numeric numeric characters.
4. Verify that the NDC you are using is valid. Methods of verification include:
   • Compare the NDC that you are inputting to the NDC that is found on the bottle, package, or product sheet for product.
   • Perform a search in the National Drug Code Directory
     1. Navigate to the National Drug Code Directory by clicking the hyperlink in this step.
     2. In the "Select Type" drop-down menu, choose "NDC Code".
     3. In the "Enter at least three characters" field, input the NDC you are receiving an error for. you can also complete a partial search by only inputting some of the characters in the NDC you are searching for.
     4. Click "Search".
     5. Compare the National Drug Code Directory data to data located on the product bottle, packaging, or product sheet. Ensure data such as "Proprietary Name", "Strength", "Dosage Form", and "Substance Name" matches
   • If you are able to locate a match for the NDC you are searching, and are receiving an error, submit a request to support for further research.
   • If you are not able to locate your NDC at all, submit a request for support and a PMP Support agent will assist you with performing this search.

• Error message you are receiving , if any.
• NDC number
• Drug name
• name of file that the NDC was submitted in
• your full name
• your PMP Clearinghouse email address
• your organization/place of employment
• the state that you are located in
• a picture of the drug bottle, packaging, or product sheet if available
